Tackling Lower Eyelid Puffiness

Lower eyelid puffiness is typically caused by sagging and tissue loss in the cheeks, which are considered a continuous unit with the lower eyelids in young individuals. Therefore, fat repositioning from the inside of the eyelid into creases along the lower border of the periorbital area, rather than removing that fat, is the preferred approach to address periorbital hollowness and puffy lower eyelids. This technique can be combined with microsurgical fat grafting (lower eyelids) and conventional fat transfer to the upper cheek.

The eyelid procedure typically involves making an incision in the upper eyelid crease to remove excess skin and fat. Depending on the individual’s specific condition, muscle and excess fat may be removed as well. For lower eyelid puffiness, fat repositioning is the preferred technique. In cases where the lower eyelid connective tissue is weak and prone to sagging, the outer corner of the eye may be lifted to a higher position than the inner corner. This not only enhances a youthful appearance but also protects against the development of a lower eyelid deformity called ectropion.

Younger patients often have concerns about inherited eyelid puffiness.
